15 Mio people, that is 20 % of the German population, suffer from hypertension and of about 4.7 Mio people with diabetes mellitus 50 % develop hypertension. Only 10 – 15 % of the patients are treated sufficiently and reach normotensive blood pressure levels. The German Association of Diabetes Education and Counselling Professions (VDBD) implemented a hypertension education programme all over Germany in order to enable patients with diabetes to blood pressure self-management and to enhance blood pressure and blood sugar control.
In 35 diabetes service centres in Germany, 473 patientes with diabetes mellitus and hypertension participated in the structured hypertension education programme. In a prospective multicentre study with a follow up-period of three years blood pressure and HbA1c levels were assessed before and annually after participating in the education programme.
Three years after implementing the hypertension education programme complete data of 280 patients could be analysed. HbA1c was reduced from 7.8 % before the intervention to 7.2 % (p < 0.001). In patients with type-2-diabetes blood pressure decreased from 153 to 150 mm Hg systolic and from 85 to 82 mm Hg diastolic. Patients with type-1-diabetes mellitus showed blood pressure reductions from before and three years after the intervention from 146 to 142 mm Hg systolic and from 84 to 79 diastolic. The three year comparison of all assessments showed lower blood pressure readings than after one and two year follow up. Three years after implementation 86 % of the patients measure their blood pressure with a high quality and controlled self-measurements were 139/77 mm Hg (mean).
Participating in the structured hypertension education programme results in an enduring improved mean blood pressure as well as better HbA1c values three years after the intervention.
