Tools to Achieve Goals - Therapeutic Patient Education”


Most of the care in chronic disease is managed by the patient himself/herself with limited interaction with the health care team. New capacities from the Health Care Professional (HCP) are required if we want to optimize this interaction. Therapeutic Patient Education (TPE) is the integration of educational activities performed by trained HCP to help the patient and his/her family to manage his/her treatment so he/she can maintain or improve his/her quality of life.


In this symposium participants discussed the role and results of experience in TPE in diabetes.

Therapeutic Patient Education was presented and discussed as an essential tool to achieve the defined goals for diabetes management.

The symposium was divided in four parts and included a discussion period at the end.

Part A - “Education Programmes in Germany – what can we learn?” (U. Mueller, Germany)
Part B - “Education for Intensive Insulin Treatment: where are we now?” (D. Cavan, UK)
Part C - “The Portuguese Diabetes Association experience: Patient Associations – Are they part of the team?” (J. F. Raposo, Portugal)
Part D - “Rethink Organization to iMprove Education and Outcomes – the ROMEO study” (M. Trento, Italy)
